MacPhail students: how are we doing? Let us know by completing our student satisfaction survey. The survey is confidential, however, anyone providing a name, email, and street address will be entered into a drawing for a chance at some great prizes!

The survey will open on April 15th!

Your personal information is provided solely for the purposes of survey prizes. They will not be sold or shared.

ALL surveys will remain anonymous. Surveys and contact data are given to MacPhail separately.

The first 200 people to fill out the survey will receive a free sticker sheet mailed to the address provided!

The prizes will be chosen randomly from the names of people who entered their addresses.
